Evidence-based intervention for chronic refractory breathlessness: practical therapies that make a difference.

چکیده
British Journal of General Practice, November 2013 609 IntroduCtIon Chronic breathlessness which is refractory to treatments for the underlying condition is a common symptom of frequently encountered conditions that present in primary care, including lung and heart disease, and cancer. Primary-care based chronic illness initiatives such as clinics for asthma or chronic obstructive pulmonary disease (COPD) and community heart nurse specialists play a vital role in optimising the management of the underlying condition(s), but for many people, especially in the later stages of the disease, breathlessness persists (chronic refractory breathlessness). Chronic refractory breathlessness is distressing for patients, their families and other caregivers, and for clinicians. Clinically, chronic refractory breathlessness is a distinct entity, rather than simply a failure to treat the underlying condition(s). It requires a different approach than that required for acute breathlessness brought on, for example, by acute coronary syndrome, an acute exacerbation of COPD, or acute pneumonia.

Non-Pharmacological Breathlessness Management in Older Patients: A Review Study
Background: Breathlessness is a multifactorial problem caused by multiple pathways and thus lends itself to a range of interventions, each targeting a specific mechanism. For a start, the aging process affects respiratory function and decreases lung elasticity and respiratory muscle strength, which results in forced vital capacity reduction. Moreover, many conditions, such as chronic obstructiv...
متن کاملThe active identification and management of chronic refractory breathlessness is a human right.
Chronic refractory breathlessness is defined as breathlessness at rest or on minimal exertion that will persist chronically despite optimal treatment of the underlying cause(s). At any time, 1% of the population report a modified Medical Research Council dyspnoea score of ≥ 3 chronically. Despite the prevalence, severity and chronicity of this symptom and an evidence base of affordable and safe...

Chronic refractory breathlessness is common and distressing in advanced disease. Despite level I evidence to support the use of opioids for this symptom, not all patients benefit. This study aimed to discover which patient characteristics predict those most likely to gain improvement in breathlessness. This is an international, multicentre, retrospective analysis of 213 individual pooled datase...
